10. MULTI-LEAGUE PLAY: The Cove Creek Park Board has adopted a rule banning play in more than one recreational league. A player whohas registered, intends to register, or intends to play in a similar recreational league will not be allowed to play at Cove Creek.
11. TEAM COMMITMENT: A player who registers to play at Cove Creek will be expected to participate in all possible team practices,scrimmages, and games. Please do not sign up if you cannot fulfill this commitment to the park and your team.
12. LIMITS ON REGISTRATION: Cove Creek Park reserves the right to limit registration in any division, at any time. Such action will take placeif any division reaches a predetermined number of registrants.
13. MISCELLANEOUS:
(a) By signing this form you are granting Cove Creek Park permission to seek emergency medical attention for your child should itbecome necessary;
(b) Every player shall be expected to return uniforms and equipment upon request from his/her coach or Park official;
(c) A registration fee of $45.00 shall accompany this registration form. Fee waivers are readily available and will be handled withcomplete discretion. Please contact Jennifer at the park office via phone or email to request a fee waiver.
